⚠️ Important Information for New Clients
Before your first appointment, please follow these steps:
Schedule an Intake Session
If you are a new client, please select an Intake Session when booking your first appointment.Complete Your Intake Paperwork
After scheduling, you will receive a booking confirmation email with links to your intake forms. Most forms take approximately 10–15 minutes to complete and can be submitted from your phone.Submit Insurance Information
Please upload your insurance card and any requested information as part of the intake process.Complete Everything at Least 48 Hours Before Your Appointment
All intake paperwork and insurance information must be received no later than 48 hours before your scheduled appointment.
❗Appointments with incomplete paperwork or missing insurance information will be canceled or rescheduled.
Completing these steps promptly helps us verify benefits, prepare for your care, and ensure your appointment proceeds as scheduled.
